    Skin Care Products and How to Use them for Every Age

    Your skin may stay with you your entire life, but you shouldn’t take care of it the same way you did when you were 20 when you are 60. Your skin changes, just like you do. And as it changes, it needs different things. By adapting your skincare routine with your age, you can ensure that your skin is getting what it needs to be healthy and look great.

    The 20’s

    In your 20’s, your skin is still buoyant and firm. It is rich in collagen, and it can rebound quickly even with poor habits like falling asleep with your makeup on. The best thing you can do for your skin at this age is to wash it every day and use a good moisturizer. You don’t need a heavy night cream or special products. Just cleanse thoroughly and moisturize.

    Your first eye wrinkles are likely to appear in your late 20’s, so you should also plan to start using an eye cream after 25. Use it every day to keep your eyes moisturized and keep the wrinkles at bay.

    The 30’s

    Collagen production starts to decline in your 30’s, and skin renewal happens more slowly. The result is that your skin looks a bit duller and a bit looser. You’ll notice that your pores don’t look as tight, and wrinkles may start to form. You’ll see them mostly around your eyes and mouth. 

    You should be focusing on good moisturizing during this time. Use a good moisturizer every day, and drink plenty of water. Continue to use your eye cream, as well. Start paying attention to your neck during this time also. Your neck ages even faster than your face, and it needs the same care you give the skin on your face.

    The 40’s

    In your 40’s, your skin is starting to become thinner and it is losing elasticity. Wrinkles are going to be more visible around your eyes, at the corners of your mouth, and on your throat. Your skincare routine will need to include anti-aging products, including a serum, a day cream, and a night cream. Of course, you’ll need to continue using your eye cream also, though you may need one with more anti-aging ingredients, such as glycolic acid. 

    You will also need to apply sunblock every day. Your skin is more susceptible to sun damage, and without sun protection, you may see an increase in age spots and wrinkles. 

    The 50’s

    Many women go through menopause in their 50’s, and that brings a lot of changes for the skin. The skin becomes even thinner and less elastic. Cell renewal slows down, and collagen production slows, which means that the skin is drier, looser, and more prone to wrinkling. 

    Choose anti-aging products that nourish the skin and encourage collagen production and skin renewal.     A PERSPECTIVE ON JUVÉDERM VOLUMA® XC

    Are you interested to try JUVÉDERM VOLUMA® XC? Do you know anyone who has been treated with it? I would like to share with you my perspective from experience treating patients with JUVÉDERM VOLUMA® XC.

    JUVÉDERM VOLUMA® XC has been available in the U.S. since December 2014. It was FDA approved for deep (subcutaneous and/or supraperiosteal) injection for cheek augmentation to correct age-related volume deficit in the mid-face. It is similar to other hyaluronic acid dermal fillers in that it produces an effect that will breakdown over time and can be dissolved with an enzyme if need be. It differs from the other JUVÉDERM® XC dermal fillers in several technical ways, but the real difference is in how it can be used and what it is used for.

    A Perspective on JUVÉDERM VOLUMA® XC
    Dr. Nello Rossi of Willo MediSpa shares his perspective from experience treating patients with JUVÉDERM VOLUMA® XC.
    Phoenix, Arizona

    JUVÉDERM VOLUMA® XC is approved for restoring volume loss. It is viscous enough to be able to lift the skin, muscle and fat in the area of injection. I tell patients that it is like injecting small amounts of Jello. This lift allows for a hollow to be re-contoured and reshaped to a more natural youthful appearance. The product works best when it has something to lift up from, such as being placed on a boney surface. From my perspective, it is an easy product to inject and mould and causes minimum swelling.

    The study done by Allergan for FDA approval of JUVÉDERM VOLUMA® XC had over 300 patients initially but only 282 patients actually started – 235 in the treatment group and 47 in the control group. The patients in the treatment group were offered treatment of their cheeks in the zygomaticomalar region, anteromedial cheek and/or submalar region. After one month, the treatment group was offered a touch-up treatment. At that point, the study followed the patient’s responses to measure the product’s effectiveness in maintaining volume correction. Averaging the three treatment areas, the mean volume of JUVÉDERM VOLUMA® XC injected in the test patients for an initial treatment and a touch-up treatment was 6.8ml. The highest volume injected for this combined initial and touch-up treatment was 13.9ml. In other words, the test subjects used an average of 7 syringes up to the maximum of 14 syringes of JUVÉDERM VOLUMA® XC in this study.

    Patients that continued in the study had their treatment response evaluated by two different methods – the Mid-Face Volume Deficit Scale and the Global Aesthetic Improvement Scale. Favorable treatment responses respectively were 85.6% and 82.2% at 6 months, 85.2% and 78.3% at 12 months, 85.2% and 78.3%, 71.5% and 68.0% at 18 months and 67.1% and 69.7% at 24 months. As you can see from the responses, as the months progressed from an initial treatment the correction started to decrease. However, this study is how Allergan makes the claim that JUVÉDERM VOLUMA® XC provides a correction for up to 24 months. They do not claim that it is as full of a correction as shortly after an initial treatment.

    Let me put this all into some perspective. JUVÉDERM VOLUMA® XC is sold by the 1ml syringe. The advertising we see on TV and in magazine ads show the results of treatment but do not really give information as to how much product may be needed to achieve these ideal results. The average patient comes in with the expectation that a “treatment” will correct their hollows, but they don’t understand the scope of how much product may really be needed to achieve that goal. On top of it all, JUVÉDERM VOLUMA® XC is a pricey dermal filler. A patient needs to be informed of real expectations and real cost.

    What has been my personal experience with patients? They have been overwhelmingly satisfied with the product. Patients report liking JUVÉDERM VOLUMA® XC and that they want more. I have had multiple patients come back in for further enhancement of treated areas.

    Because of cost, treatment to restore volume to a patient’s cheeks may need to be done in several sessions. Adding or building up on existing layers of filler is common and practical. Most patients don’t want to look over-inflated but would rather see a gradual improvement over time. Because of the duration of a treatment result, a touch-up should be needed less often than with other dermal fillers, thus offsetting overall maintenance costs. Looking at test subjects’ favorable responses in the Allergan FDA study, patients probably should think about doing a touch-up treatment somewhere between 12 to 18 months.

    In the U.S., JUVÉDERM VOLUMA® XC is approved only for mid-face age related volume loss. World-wide, JUVÉDERM VOLUMA® XC has been used to treat many different areas that were not FDA approved for treatment here, such as temples, chin, nasolabial folds, nose, lips, dorsal surface hands, nipples and glans penis. Though it is not FDA approved in the U.S. for Facial Lipoatrophy caused by HIV, a study done in Europe showed 100% of 21subjects reporting a much improved or a very much improved result at 9 months post-treatment with JUVÉDERM VOLUMA® XC.

    To summarize, JUVÉDERM VOLUMA® XC is a great product for what it can do. It is a costly product, so make a treatment plan that includes an initial correction and periodic maintenance touch-ups. Have a discussion about your aesthetic goals and the realistic results that can be achieved within the constraints of your budget so that you will not be disappointed.

    OK. You’ve had a BOTOX® treatment to get rid of that constant frown. You’ve used JUVÉDERM VOLUMA™ XC to augment your hollow cheeks and JUVÉDERM® XC to soften your nasolabial folds and to augment your lips. You’ve been using LATISSE® to enhance your eyelashes. So why are you still using facial cleansers and products that you are getting from QVC or Target or Macy’s or that online auto-ship brand your friend is selling? Really? You need proven medical grade products to enhance your skin and protect your investment.

    Medical grade skin care products have higher concentrations of active ingredients in purer formulations. Medical grade skin care companies don’t sell their products in department stores or through the website hosted by your friend. They are available through medical offices. Why do the sales people in skin care at department stores wear white coats? It’s a marketing trick to fool you to think that they are knowledgeable medical professionals selling the products.

    At Willo MediSpa, we have tried several medical grade skin care companies since opening in 2008. We have migrated to Neova® and SkinMedica® skin care products that we feel give the best results to correct and maintain your healthy skin. SkinMedica® uses the GRASS method to put together a skin care regimen that is right for you. This acronym stands for Growth Factors, Retinol, Antioxidants, Speciality Items and Sunscreen. It is also the order in which you apply your products.

    I would like to discuss four different skin types and the products and treatments we have to offer. Please keep in mind that some skin issues need more aggressive treatments than we currently offer at Willo MediSpa. There are other medical offices that offer laser and resurfacing treatments. Also, there is no one product line that everyone feels is best for them. But if you are like most, a good skin care regimen of medical grade products with an occasional chemical peel and/or microdermabrasion will keep your skin looking healthy and vibrant.

    DRY/AGING SKIN TREATMENT PLAN

    This is probably the most common skin issue I see with my patients – both male and female. Your skin is dull and flat with a rough texture. Your skin doesn’t appear radiant or vibrant. It detracts from the overall appearance of your face. You spent years in the sun with no sun protection. You might use a product from here and there that you saw advertized in a magazine but don’t follow any daily regimen. The objective of this treatment plan is to restore the youthful appearance of your skin.

    Below are the products and treatments we recommend and offer at Willo MediSpa using the GRASS method. This treatment plan would start with a correction phase which would include a series of SkinMedica® chemical peels and an occasional microdermabrasion to boost the home regimen results. The home regimen would include a cleanser that helps with daily exfoliation such as Neova® Radiant Cleanser, SkinMedica® TNS Essential Serum to help correct fine lines by building up you own natural collagen, SkinMedica® Retinol for nightly cell turnover, a moisturizer/antioxidant such as SkinMedica® Dermal Repair and a broad spectrum SPF sunscreen such as Neova® DNA Damage Control – Silc Sheer 2.0 or Active. A specialty product to consider would be either SkinMedica® TNS Eye Repair or Neova® Illuminating Eye. As you start to see the results you want, your maintenance phase would consist mostly of the home regimen with an occasional SkinMedica® chemical peel or microdermabrasion.

    Skin Care Treatment Plan for Dry and Aging Skin

    PIGMENTATION SKIN TREATMENT PLAN

    This skin issue is very common. Women are more affected by pigmentation since their hormones play a part in the presentation of melasma and colasma. Men and women can be affected by post-inflammatory hyperpigmentation which can be a result from a scar or an acne breakout. The objective is to even out the skin tone and lighten the discoloration. Pigmentation issues can be very stubborn and take a long term treatment commitment.

    Below are the products and treatments we recommend and offer at Willo MediSpa in Phoenix using the GRASS method. This treatment plan would also start with a more aggressive correction phase using a series of SkinMedica® chemical peels such as the Vitalize Peel® or the Rejuvenize Peel™ and microdermabrasions to boost the home regimen. Your home regimen would include a cleanser such as Neova® Radiant Cleanser, SkinMedica® TNS Essential Serum, SkinMedica® Retinol, SkinMedica® Lytera for lightening skin discolorations and evening out your skin tone or using the stronger Neova® Complex HQ Plus which has a 4% hydroquinone. In your maintenance phase, a periodic chemical peel and/or microdermabraison will enhance your results even more. It is of utmost importance when dealing with pigmentation issues to protect your skin with a broad spectrum SPF sunscreen such as Neova® DNA Damage Control – Silc Sheer 2.0 or Active. Going through the process of lightening the pigmentation would be for naught if you continued to expose your skin to the damaging effects of the sun.

    Pigmentation Skin Care Treatment Plans For Phoenix Residents

    OILY/ACNE SKIN TREATMENT PLAN

    This skin issue can affect us as teenagers or as adults or our entire lives. I’ve had patients tell me that as a teen they had perfect skin, but as an adult they have developed oily/acne skin. Severe acne should be treated by a Dermatologist at any age, and there are times when oral medications are helpful. Mild oily/acne skin can be treated with skin care products designed for that purpose and with products that won’t congest your pores or promote breakouts. Retinols and salicylic acid products are the standard for treating acneic skin.

    Below are the products and treatments we recommend for treating oily/acne skin at Willo MediSpa using the GRASS method. In this treatment plan, Neova® Purifying Cleanser for daily use and Neova® Breakout Swabs for spot treatments contain salicylic acid. The antioxidant component of SkinMedica® TNS Essential Serum may be too heavy on this skin type. Instead, SkinMedica® TNS Recovery Complex and the lighter SkinMedica® Ultra Sheer Moisturizer are recommended. A nightly SkinMedica® Retinol is recommended and comes in three strengths – 0.25, 0.5 and 1.0. This allows you to select a retinol that works for you without producing side effects. As always, a broad spectrum SPF sunscreen such as Neova® DNA Damage Control – Silc Sheer 2.0 or Active – is recommended, especially to help inhibit post-inflammatory hyperpigmentation. SkinMedica® chemical peels, which contain salicylic acid, can be used to help control and maintain acne prone skin. Neova® Smoothing Gel containing 10% glycolic acid is an alternative for patients allergic to salicylic acid.

    PREVENTATIVE SKIN TREATMENT PLAN

    You’ve been lucky and smart. You inherited the good skin genes from your parents. You’ve limited the damage caused by the sun to your skin. You consistently use sunscreen and regularly use moisturizer. But you are noticing things are starting to slip just a bit or you realize that you don’t want them to start slipping. Preventing skin issues will cost less over time than correcting them after they have set in.

    Below are the products and treatments we recommend for a simple preventative skin care plan at Willo MediSpa using the GRASS method. Probably the hardest part of this kind of treatment plan is realizing that there will be a daily routine that you will want to follow just like brushing and flossing your teeth. Neova® Herbal Cleanser is gentle and won’t dry your skin. SkinMedica® TNS Essential Serum will help keep away the fine lines that are a sign of aging skin. Nightly SkinMedica® Retinol will help enhance skin texture. SkinMedica® Ultra Sheer Moisturizer will help keep your skin hydrated – something very important in our desert climate. The better sunscreens are now called broad spectrum SPF sunscreens and protect your skin from both UVA and UVB rays. We carry Neova® DNA Damage Control Silc Sheer 2.0 SPF 40 and Neova® DNA Damage Control Active SPF 43.
